Both blogs reflect ongoing efforts to better understand current practices, identify gaps, and explore opportunities to support cardiac rehab programs in practical, scalable ways.<\/span><span style=\"font-family: Arial;\"><\/span><\/p>\n<p><span style=\"font-family: Arial;\">Finally, we shared a <a href=\"https:\/\/www.youtube.com\/watch?v=lqAUTVQ9sTQ\"><strong>short patient story video<\/strong><\/a>, offering a preview of MiCR\u2019s upcoming <a href=\"https:\/\/michigancr.org\/join-micrs-new-heart-to-heart-storytelling-initiative\/\"><strong>Heart to Heart<\/strong> <\/a>project. This initiative aims to capture and share patient experiences with cardiac rehab, stories that remind us why this work matters and ground our improvement efforts in the voices of those we serve.<\/span><\/p>\n<p><span style=\"font-family: Arial;\">Data and reporting also remain central to our improvement efforts. Since launching cardiac rehab reporting in 2020, participating sites have tracked steady gains in CR utilization, improvements that translate to estimated lives saved and readmissions avoided.
