As Cardiac Rehab Awareness Week comes to an end, we wanted to take a moment to reflect on the conversations, stories, and resources shared throughout the week, and what they represent for cardiac rehab programs, providers, and patients across Michigan.
Throughout the week, the Michigan Cardiac Rehab Network (MiCR) highlighted a set of core web offerings on MichiganCR.org that support cardiac rehab at every stage, from identifying a program to strengthening care delivery and staying connected as a community.

- Best Practice Toolkit: A cornerstone MiCR resource supporting quality improvement in cardiac rehab, offering evidence-based strategies across key phases: from initiation to innovation, to improve enrollment, attendance, and patient outcomes.

New this year is the Cardiac Rehab Scheduling Calendar, developed through a collaborative brainstorming session at the 2025 MSCVPR meeting with partners at HBOM.
- News & Events: A central hub for MiCR updates, upcoming meetings, partner events, and opportunities to stay connected through the MiCR mailing list, helping strengthen communication and collaboration across the network.

In addition to these web resources, MiCR shared two new blog posts during the week, one focused on our Medication Management strategic initiative and another providing an update on the Cardiac Rehab Telehealth Survey. Both blogs reflect ongoing efforts to better understand current practices, identify gaps, and explore opportunities to support cardiac rehab programs in practical, scalable ways.
Finally, we shared a short patient story video, offering a preview of MiCR’s upcoming Heart to Heart project. This initiative aims to capture and share patient experiences with cardiac rehab, stories that remind us why this work matters and ground our improvement efforts in the voices of those we serve.
Data and reporting also remain central to our improvement efforts. Since launching cardiac rehab reporting in 2020, participating sites have tracked steady gains in CR utilization, improvements that translate to estimated lives saved and readmissions avoided. MiCR’s cardiac rehab push reports were highlighted by Million Hearts as a model for hospital-level reporting nationwide, and several health care systems have leveraged custom analytics to further evaluate performance and identify opportunities for efficiency and growth.
Cardiac Rehab Awareness Week offered an opportunity to pause, reflect, and reconnect, with each other, with our shared goals, and with the patients at the center of cardiac rehabilitation. Thank you to the programs, partners, and individuals who continue to contribute ideas, tools, and stories that strengthen cardiac rehab across Michigan.
